
面对疫情防控新形势,做好偏远山村重点人群的健康管理意义重大,浙江依托数字化发展优势,用数字手段破解山区农村医疗卫生服务“最后一公里”的难题,形成了山区基层疫情防控“筛、防、管、治”闭环服务体系。
位于浙西山区的丽水龙泉市下辖行政村 236个,其中偏远山村173个,占比73.3%,是典型的山区县市,山高路远,地广人稀,基层医疗资源不足、村医配置不全、山区群众看病难等短板明显。结合自身区域特点,龙泉市依托已有的“浙丽乡村好医”医疗健康应用体系,上线新冠重点人群疫情防控应用模块。
在兰巨乡炉田村的文化礼堂中,村里的健康协管员陈汉青正在为阳康老人邓根福测量血压、血糖、血氧饱和度,监测结果同步上传到数字化应用管理后台,后台家庭医生发现数据异常时便能及时介入跟踪随访。
陈汉青说,村里老年人比较多,大部分都有基础疾病,离最近的乡镇卫生院也有近十公里山路,有时遇到我们处理不了的问题,可以在平台上连线医生实时远程问诊,确定是否按照分级诊疗规则入院或转院,让感染新冠的老人安心养病、没有感染的做好防护,同时避免轻症转重症、重症无人管的情况发生。
据统计,2022年12月1日至2023年1月30日,龙泉市山区居民通过智能一体机、便携设备完成日常监测1977人次,派发健康协管员、家庭医生主动干预任务共1062人次;通过“健康大脑”算法模型主动派发远程问诊任务90人次,有效防止新冠防疫重点人群25人感染转成重症的风险。
中医药在疫情防控工作中有独特的作用和优势,也受到不少老年人的认可。春节前夕,家住台州市仙居县官路镇下王村的88岁老人俞万章收到了快递到家的中药,自今年初老人感染后持续咳嗽,在家人的陪同下到仙居县中医院官路院区就诊,中医师为他开具 “清肺排毒汤”治疗。
“在镇里中医馆看完病后,我们便先把老人送回家,后续煎药、配送都在‘云上中医’平台上查询,各个环节一清二楚,最后还能快递到家,这种数字化平台确实方便。”俞万章的儿子俞新忠说,父亲住在偏远的乡村也能在家门口及时收到药包。
“朱医生,您来啦!”“老毛,吃了药好点没?”1月31日一早,杭州市临安区西天目卫生院的“签约服务随访车”停靠在天目山镇武山村独居老人毛宝林家门口。
今年68岁的毛宝林自小患小儿麻痹症,2022年又查出肺癌晚期,由于住在山区,毛宝林想去离家11公里外的镇卫生院都是一种“奢望”,朱雯凯的经常到访,让老毛的困境得以扭转。
朱雯凯2022年7月份开始担任武山村辖区内的签约医生,每周二、周四便会把心电图、血压、血糖等多功能诊疗包配备齐全的随访车开到患病村民家门口,提供入户复查、配药、随访等一系列服务。
“去年新冠感染高峰时,老毛也感染了,我一直密切关注着他的情况,每周两次下村,只要在附近都会去看他。”朱雯凯说,还好他的症状比较轻,用些常规抗病毒感冒药就好转了。
杭州市临安区卫健局副局长陆旭军介绍,全区除了配置20量携带常规检查设备的“签约服务随访车”,还同步配置5辆带有DR、心电、超声等仪器的“综合巡回诊疗车”,通过5G网络,开通远程诊断、巡回医保结算。疫情期间还利用这些“流动诊室”开展老年人疫苗接种、配药送药、用药指导等服务,让新冠居家的老年人、失能半失能、孕产妇、儿童等特殊群体更加安心。
